After you purchase your lenses, please fill out this form with your prescription information. Once we receive your prescription information, we will fulfill your order. If you have any questions, please contact us here.

* Prescription Kit does not come with Vuzix Shield smart glasses. Vuzix Shield smart glasses will need to be added separately.
* The Prescription Kit will be shipped within 4-6 weeks of your order date. The Vuzix Shield smart glasses and Prescription Kit Inserts will be shipped to you in 2 separate shipments.
